The Association of New York Health Care Facilities sponsors a New York State Insurance Fund Safety Group for nursing homes, assisted living facilities, and residential care centers.

In order to participate in the Safety Group, the following minimum requirements must be met:

1.) the employer must be a member in good standing of the Association
2.) the employer's governing class code must be 8866 or 8829
3.) the employer's 5-year cumulative incurred loss ratio cannot exceed 40%
4.) the annual premium size should exceed $30,000
In order to process your application, the following documents must be completed: NYSIF Application (attached PDF file); Membership Confirmation form (attached Word document); and a Service Fee Agreement (attached Word document). You or your insurance broker should send these documents to our Group Manager, The Hamilton Wharton Group, Inc., along with 5 past years of loss runs and premium audit statements.
